Q: The forms I have tried all work perfectly if I choose a single source and save as php, but when I try and save an html file as apply.htm and the php as apply.php the form always gives a 404.
A: Is the 404 error when you try to view the form (.htm) or when your submit it? If it occurs when your view the form then check your server as it is simply an upload naming issue. If it is when you submit then check the form action in the htm file to ensure that it points to the php file ie action="apply.php". If you still have problems try calling apply .php directly from your browser to make sure it exists, if there is no 4040 error then try changing the name of apply.php to something different from the htm file name in case your server ignores file extensions ie applyprocess.php (make sure you also adjust the form action to match).

A: Your Registration Details only work with the version software you purchased. You purchased version 11 so you must use your registration details with version 11. It appears you are trying to register version 20. I retested your registration with version 11 software and it is fine. Download and install version 11 software from https://bizpep.com/superseded.html . The software version number is displayed at the top of the help file (2nd line).

A: I am not sure what you mean by "tried to get the Form1 Builder Software" and "does not work", but if cookies are deleted, or removed (when something happen to your computer?) you will need to re-input your registration details to re-register your Form1 Builder before you can build registered forms. Cookies must be enabled to record the registration details. If you now have an old browser you will need to update your browser, you can do this for free, for details see https://bizpep.com/form1/ . Are you trying to register the correct version software ie the version prior to Aug 2004 is available at https://bizpep.com/superseded.html . If none these not solve the problem please provide more detail.
