Q: I started my computer over again and I tried to get the Form1 Builder Software. I tried it many times and my resisted info doesn't work?
A: I am not sure what you mean by "tried to get the Form1 Builder Software" and "does not work", but if cookies are deleted, or removed (when something happen to your computer?) you will need to re-input your registration details to re-register your Form1 Builder before you can build registered forms. Q: I have my form - sending a csv file with the email - how can I make the csv file have column headers with specific titles I will give them?
A: The csv function is designed to send only the data component of the form because usually multiple form submissions will be combined into a single csv file by the receiver. You can use headers by setting up a csv file with your headers required (in the order of data sent in your form) and then importing the delivered csv form data files into the master file when data is received. This allows you to import multiple form data submissions into a single file.

Q: I purchased form builder and created the form as defined in the software. I saved the code in notepad as a .php. Now what? I do not see directions on what to do next. I know the file must be uploaded to my server, but....I want retailers who visit my site to click on a button on the main page that brings up this form and once filled out it is sent to me. Where are the directions on what to do after the .php file is created?
A: The form file is like any web page file. Upload it to your server, input the url (http://www.domainname.com/form.php) in your browser (ie Internet Explorer) and it will appear. Link to form from another page exactly as you do with normal web pages, the form is simply a web page with some added code ie something like <a href='http://www.domainname.com/form.php'>Click for Form</a>.

Q: I have recently purchased a licence to use your software. Tonight I built another form, only to find it has switched back to an evaluation version. How has this happened? And how can this be rectified? I have already used the registered version successfully and would like to continue to do so.
A: The registration details are recorded as cookies on your computer. If your cookies are cleared or removed you will loose the registration details and will need to re-input them to build a registered form. To maintain the registration details entered ensure cookies are enabled and are not cleared.
