A: This is most likely due to not setting the Defined Email AddressForm Field see the Help file section on Defined Email Address FormField to set this. You may also want to set the Defined Name fields.

Q: When the form was tested on a remote computer, who's user has autocomplete for forms set up, my name and email address are automaticallyfilled into the form?
A: This is probably because the form html code was copied from a webpage that was viewed in your browser with a Google automatically fillin the form data function (usually in a Google toolbar). This toolbarmodifies the html code and if you copy that code the form values areset as your information. If you view the source of the page (or openit in a text editor) you will see the form field values are set withyour data. Remove the values and the data will not be there.

A: Form1 processing processes all Named form fields. If you do notgive it a name in the html then it will not be processed, ie use<input type="submit" value="Send My Request" /> .

A: The relative indicators are about looking forward (the previousyear values are just a starting point). The Relative Indicatorsconsider the things that are going to impact on the business. Considerthe environment in which the business operates and theopportunities/threats that are present. What is the impact of these inyear 1, 2 etc. If for a specific Relative Indicator everything willremain the same then use 100%, this indicates that this is the same asthe past year.
