Q: Does the 'For URLs with' box allow, for example, index.php to be identified by the word 'index' or 'index.php' or does there have to be a word embedded in the page as body text or head or pre-head content?
A: The For URLs with value is a character combination included within a url ie if the url is For URLs with value then this will use all urls with index in them ie http:// www.mydomain.com/index.php or http:// www.mydomain.com/someotherindex.html etc. It only uses the url no page content to identify a match. So index.php will be identified by the word 'index' or 'index.php' but not 'index.html'.
